Manajemen Anestesi pada Laki-laki 47 Tahun dengan Syok Sepsis yang Menjalani Operasi Amputasi Pengendalian Sumber Infeksi
Background:Sepsis is a systemic disorder as a result of the interaction between the immune system and infectious agents. In some cases, sepsis can progress to septic shock and death. In patients with sepsis with ongoing bacterial contamination, resuscitation may not be successful until control of the source of infection is established. Case:A 47-year-old malewith septic shock and diabetes has been intubated in the intensive care unit undergoing amputation surgery to control the source of infection. From the previous history, it was revealedthat there was a history of an unpleasant smelling wound on the left footthat was difficult to heal, extended to the knee, thenwas admitted to the hospital with complaints of shortness of breath then was admittedto the intensive care unit. From physical examination revealedleft footulcer that extended to the knee.Discussion:General anesthesia was performed with a combinationof ketamine, fentanyl and rocuronium. Patient was successfully extubated on the 3rdday after surgery,then moved to the general ward2 days later. Conclusion:Anesthesia in septic patientsmayhave deleteriousimpact that requires careful planningin everystep. Anesthesiologists have an important role in providing anesthetic management to optimize better patient outcomes.Keywords:amputation; anesthesia;ketamine; sepsis;source control
